New swimmers registering with Cardinal for the first time must fill out the following forms and provide a copy of swimmers birth certificate!

A free 2 week trial period will be available to new families registering, but you will still be required to complete all forms and make the first payment.  If your swimmer chooses not to continue with CARD, refunds will be made after the 2 week trial.  *Note: New High School Satellite swimmers do not have to complete the Payment Option Form, but must complete all other forms.
